SS-31, also known as elamipretide, is unusual in the research peptide landscape for one significant reason: it has been through genuine phase 2 and phase 3 human clinical trials across multiple conditions. Most research peptides in this space have animal data and limited human evidence. SS-31 has substantially more, which is why it merits serious attention.
SS-31 works differently from other mitochondrial peptides in the library. It targets cardiolipin — a phospholipid found specifically in the inner mitochondrial membrane that is essential for the proper functioning of the electron transport chain. The electron transport chain is the process by which mitochondria generate ATP.
With age and metabolic stress, cardiolipin undergoes oxidative damage, which compromises electron transport chain efficiency and increases mitochondrial reactive oxygen species production. SS-31 stabilises cardiolipin, protecting the inner membrane and restoring electron transport chain function. It concentrates in the mitochondrial membrane at concentrations far higher than in surrounding tissue — it is effectively a mitochondria-seeking molecule.
SS-31 has been studied in heart failure with preserved ejection fraction, Barth syndrome (a rare mitochondrial disorder), limb girdle muscular dystrophy, and age-associated muscle weakness. In the heart failure trials, SS-31 improved exercise capacity and quality of life measures in patients with mitochondrial dysfunction as a contributing factor. In Barth syndrome, it showed improvements in walking distance and cardiac function.
These are real human outcomes from real clinical trials — not animal models extrapolated to humans. The evidence standard for SS-31 is genuinely higher than for most research peptides.
Research in older adults has shown that SS-31 improves mitochondrial respiration in skeletal muscle and may counter the age-related decline in muscle energy metabolism that contributes to sarcopenia (age-related muscle loss) and reduced exercise capacity. This has led to interest in SS-31 as a research compound for both athletic performance and healthy aging.
